Read and sign the following
information: New applications must send photos
of your work and display. Please pre-pay with
Visa/Mastercard, or check / money order and a self-addressed stamped envelope must
accompany all applications. If the show is full
your payment will be returned. If you cancel within
2-weeks of the show no refunds or credit will be
issued.
NO IMPORTED ITEMS, MASS PRODUCED ITEMS AND/OR
KITS.
I understand that Artist To You LLC, the sponsoring organizations, owners or
managers of the show site are not responsible for
lost, stolen or damaged property or equipment, nor
accident and/ or injury to me, any staff or other
persons that I may have in my booth, and do hold
harmless any of the above mentioned individuals of
any and all liability. I also understand that if I
show products not approved by Robert Staniford,Wanda MacioszekI
will be asked to leave the show and no refund will
be given. I understand a 10% commission of all
gross sales and orders taken at the show will be
given to the promoters at the end of the
show. I understand and agree to all of
the above requirements and conditions of this
application. |
